SchemeWise staff have, collectively, many years experience in the community, health, social and disability sectors.  SchemeWise came into being in response to personal lived experience of disability – from which we know and understand the full range of disability issues.

SchemeWise people proactively practice and put YOU at the centre of everything they do by upholding and working in a human rights compliant manner and the NDIS Code of Conduct. Our only desire is to support you (your family or representative and/or other supports) by developing a comprehensive and unique model around capacity building through Support Coordination and Therapy services.  SchemeWise is a dedicated business for this purpose.

We use our health and allied-health knowledge across mainstream, health, mental health, justice, child, disability and community sectors to help you achieve, manage, coordinate and build your best life!

SchemeWise is a no surprise, transparent and openly accountable service built from the ground up to specialise in its capacity-based service roles. We use specialist systems purely built for these functions and practice continuous improvement and quality processes so we remain up-to-date with NDIS changes, placing person-centred practice, your goals and your right to choice and control front and centre. You can choose one or however many of our services you think are best for you....  We also love working with your other providers!

All our people hold clearances to work with vulnerable people (ACT) and children (NSW), and have professionally registered qualifications.  In this way, we offer you very high quality-assured services backing up our expertise. We also take safety very seriously in everything we do, and we want to ensure you receive the best supports possible. 

SchemeWise works with all kinds of sectoral people like Local Area Coordinators (LAC’s), service providers, hospitals and the NDIA itself, to design services to fit YOUR needs.  SchemeWise services are designed to complement how you live your life – because YOU are at the centre of everything WE do.

 SchemeWise offers no obligation meetings to discuss your NDIS plan and support needs.  We are only too happy to tell you about SchemeWise services and YOU get to choose whether they will work for you.  Our friendly team can help you with NDIS Support Coordination (to specialist level), Complex Planning or Therapy services (nursing, counselling, recovery coaching, OT, and physiotherapy services)… all done the right way!

SchemeWise is also a proud disability employer adding meaningful opportunities for people with disability to contribute to society and help others be their best selves.  With this in mind, the SchemeWise team offers deep knowledge of the NDIS from the participant, representative, and provider perspectives.

Our team of professionals are central players in SchemeWise person-centred approach in a multidisciplinary way. They provide the right scaffolding to meaningfully work with a broad range of people from moderate, through to very complex needs, and have interactions with various NDIS services teams including the Hihg Needs, and Complex Network pathway. Most importantly, our team are experienced across mainstream interfaces including; health, mental health, social, justice, child protection, public/community housing, plus community social and disability services.

✅  Committed to delivering services and supports in partnership with individuals, their family/representatives and their broader supports network.

People can access a SchemeWise support either as a single service, (e.g. support coordination), or a few services (like nursing and counselling, or, support coordination and counselling) if people wish to choose us.  Whether you choose one or more services, SchemeWise people will be there to assist and back you up, to help when needed, and build your capacity toward a better life. Helping you achieve your goals is what we do.
