Our health professionals include occupational therapists, physiotherapists, registered nurses, counsellors and case managers to help guide your journey toward a better tomorrow - supported within your preferred living arrangements. They can also help with improving life and personal skills, or with carer strategies to manage cognitive problems, providing reliable and trusted support in your home or retirement village, or even within care facilities too.

SchemeWise is a specialist neurological provider across Canberra and Queanbeyan helping people with issues from physical functioning to brain, spinal cord, physical and associated psychosocial problems keep well and live with dignity. And yes - we work with people and train our people to provide trusted specific dementia supports!

Our nurses work with carers and families to understand the changes that advancing age can bring, and our counsellors can help people to adjust to changes in their circumstances, think and plan for their future and with carers or care teams too. Our physiotherapists can help with gentle guided-exercise and improving mobility, while our OT's can provide recommendation / support for assistive technology, cognitive needs and safety planning.

  • advising about modifications to your home/facility environments or daily activities to reduce pain, fatigue or improve general functioning, to make your life easier and get back to the things that are important to you
  • working with you and your doctor to design a plan to help you stay well at home
  • supporting daily life activities, so you can live independently and maximise health, wellbeing and safety
  • supporting you to plan for your future needs (or for when you need extra help)
  • we also welcome the opportunity to work alongside, and with, your other trusted supports and services.

There are a number of people you may come in contact with during your twilight journey. Our case managers and allied-health professionals can help you navigate through these systems and schemes to achieve your full support potential.
