This helps ensure we get your service(s) right... from the start.

Getting started together is easy!

We'll make a service agreement, so you know the cost of your service and how it will support you.

Service agreements are a CONTRACT. Each party must do certain things, and meet their responsibilities at all times.

We'll need to know if your service will be plan-managed or self-managed (how you'll pay for your service) - we add these details to the agreement too.

If you have a plan-manager, you must tell us who that is and provide their contact details.

We'll then complete a service plan together, so you can tell us how we should help you achieve your goals.

We'll review your goal progress together, regularly, and make adjustments to your service that you wish.

To help us, help you... we will require some background info and documents.

We'll need a recent photo of you for your customer file (like the ones below).

This information makes sure we know who you are, understand your health & disability needs, and what has happened in the past.

Your information is kept private & confidential at all times.
