NDIS Support coordination is a service for participants who have the ‘Coordination of Supports’ line item in their plan and who choose to plan-manage or self-manage their plan funds. This service is particularly useful for people living with neurological, cognitive, physical, brain and spinal injury conditions, or who may have low informal supports, or more complex needs.

This SchemeWise service is available in the ACT region including Canberra and Queanbeyan, plus Wagga and Albury areas.

SchemeWise support coordination services are a compassionate and valuable resource for people which offers experienced and clinically qualified health and allied-health people (not certificate level quals) providing customers a very high level of assurance and quality, compared to most other providers. It is a person-centred service designed to meet the specific needs of each person, in consultation with them. The service can also assist plan nominees, legal guardians or power-of-attorneys through their role  supporting a participant.

Our people don’t just get NDIS… they know how other service sectors and related transitions work in context with it – which can be invaluable when a person is moving on after school, seeking work, lives with a chronic condition and may often go to hospital, or need support with other service sectors such as housing, vocational training, health/mental health, or multi-services needs. The service also seeks to build people’s personal capacity and knowledge (e.g. of how each NDIS service works and the guidelines they are required to follow).

Our people have lived experience with disability too, and some have family members who participate in the NDIS also. What will this service do?

SchemeWise support coordinators work closely with NDIS participants to develop an individualised plan that aligns with their NDIS goals. They help participants to understand their NDIS plan and funding, connect with service providers, and access the specific supports they need. Our support coordinators are trained professionals, backed up through government registrations with AHPRA providing people expert guidance and advice on how to navigate the NDIS system, as well as access supports available to them.

Support coordination services are person-centred, meaning that they are focused on the individual needs and goals of the person. This approach ensures that participants receive tailored support to achieve their goals and live their lives to the fullest.Our support coordinators also help with monitoring services and supports, making adjustments and with transitions between providers too. And they can help with housing exploration such as moving out of home into a private rental, moving into a supported group setting and with SDA planning.

It is important to recognise that support coordination is only provided to around 30-40% of NDIS participants; generally for those who require additional assistance in accessing the supports they need. Support coordination services can help you understand your NDIS plan and funding, connect with service providers, and access the specific supports you need to achieve your goals.

Support coordination services are available to NDIS participants who have a support coordination line item in their plan. This funding must be plan-managed or self-managed for SchemeWise to be able to assist (if funds are NDIA-managed then participants can make a request to NDIS for a plan variation to change their funding to the required types). The focus of Level 2 general coordination services are directed to implementing the NDIS plan as funded, and relevant to, the person’s stated goals.

Specialist coordination services

Specialist support coordination services are available for participants with more complex needs (plan- and self-managed only). It may only be offered to NDIS participants who identify a complex or risk problem. This higher level service is intended to provide more intensive support to participants who require additional assistance in navigating the NDIS system, who may have multiple system touchpoints or complex needs, may live with particular higher level risks (e.g. unstable housing), and/or trouble accessing the specific supports they need.

SchemeWise specialist support coordinators work closely with other service providers (including general level coordination teams) to ensure that participants receive expert and integrated care planning and support as well as developing services to meet complex needs. This service is often given in a time-limited way be the NDIS (e.g. for 50 hours total, plus with a further general level support period for use once complex works are finalised).

To provide specialist level servicing, each coordinator must hold relevant university health/allied-health qualifications, have appropriate scope-of-practice and experience, participate in ongoing professional development as required by their profession, and be a member of a professional body under the NDIS suitability rules. The service may be delivered by a registered nurse, physio, OT or rehab counsellor at SchemeWise.

NOTE: People accessing the justice system MUST have available specialist support coordination as the court or tribunal system will generally appoint a case manager / registrar and there are formalised processes within the judicial system for which interaction and involvement are necessary. Often coordination will also involve assisting the person, or their legal support (or seeking the person gain such representation), and/or sourcing relevant health or other information too (particularly if the person is unable to do so themselves). Coordination may also seek involvement and connection with an advocacy service. Coordination cannot ‘represent’ a person in any judicial matter, only support them through the process. Person’s in custodial settings transitioning back to community life, MUST have specialist coordination in the first instance to assist ongoing judicial interaction, plus community settlement and services connection. They may also require multiple systems access across judicial, housing, health, mental health, community and disability services/supports.
